At Open Goal Soccer, the safety and well-being of every child is at the heart of what we do. While our programs are designed to be supportive, safe, and developmentally appropriate, soccer — like all physical activity — carries some risks. By registering your child in our programs, you acknowledge and accept those risks, while trusting us to create the most secure environment possible.

 

1. Assumption of Risk

I, the undersigned parent/guardian, understand that participation in soccer activities involves physical activity and may include, but is not limited to: running, kicking, collisions, falls, and outdoor play. I acknowledge that injuries (including minor bumps and bruises, or in rare cases, more serious injuries) may occur. I voluntarily allow my child to participate, fully assuming all risks associated with these activities.

 

2. Medical Authorization

In the event of injury or medical emergency, I authorize Open Goal Soccer staff, coaches, or volunteers to seek emergency medical care for my child as needed. I understand that every reasonable effort will be made to contact me first, and that I am responsible for any medical expenses incurred.

 

3. Release of Liability

​

In consideration of my child’s participation, I release and hold harmless Open Goal Soccer Inc., its directors, coaches, staff, and volunteers from any and all liability, claims, or demands arising out of or connected with participation in these programs — except in cases of gross negligence or intentional misconduct.

 

4. Safety and Conduct

 

I acknowledge that my child is expected to follow safety rules, listen to coaches, and participate respectfully with peers. Open Goal Soccer reserves the right to remove any participant whose behavior jeopardizes the safety or experience of others, without refund.

 

5. Agreement

 

I confirm that:My child is in good health and able to participate in soccer activities.I have read and understood this waiver.I voluntarily agree to its terms, knowing that Open Goal Soccer prioritizes safety, inclusion, and care in every program.
